A pre-visit questionnaire alone does not tell us you have arrived.
When you arrive at the fever clinic entrance, you must complete the same-day check-in.
You may fill in the questionnaire before your visit or at check-in on the day. You will need your name in hiragana, the mobile phone number you will use at check-in, your date of birth, and answers about your symptoms.

The check-in page is in Japanese. Here is what each part means.

Time slots are a guide, not an appointment
The time slot you choose is only your expected arrival time. It is not an appointment time. Patients are generally seen in check-in order, but the order may change depending on how severe the symptoms are.
- Weekdays (Mon, Tue, Wed, Fri, Sat): 正午まで (morning reception until 12:00) or 14時以降 (afternoon reception 14:00–18:00). Closed on Thursdays.
- Sundays and national holidays: 10時〜15時 (10:00–15:00, no lunch break), fever clinic only. See Sunday and holiday fever clinic.
After you check in
- Please wait in your car in the parking lot.
- We send an SMS when your check-in is complete, and another SMS when your turn is near.
- When your turn is near, come to the fever clinic entrance, press the intercom button and say your name.
- If you wait a long time without any message, please open the check-in page again and make sure you completed the check-in.

2. When your turn is near
診察の順番が近づきました。発熱外来入口までお越しください。 — “Your turn is near. Please come to the fever clinic entrance, press the intercom button and say your name.”
Frequently asked questions
- If I choose a time slot, will I be seen at that time?

No. It is not an appointment. It is only your expected arrival time. Patients are generally seen in check-in order, and the order may change depending on how severe the symptoms are.
- Will filling in the questionnaire in advance get me seen sooner?

No. It only saves time at check-in. If you filled it in beforehand, you only need to confirm or correct your answers when you check in.
- Is the pre-visit questionnaire enough?

No. It does not tell us you have arrived. You must check in at the fever clinic entrance.
- I cannot read Japanese. What should I do?
